Privacy notices

Our three Surrey clinic locations are operated by two separate companies. Choose the notice for your clinic.
Manvir Purewal Physiotherapist Corp.
What we collect and why
We handle information needed to identify and contact you, arrange appointments, provide and document care, coordinate authorized services, manage billing and insurance claims, and meet recordkeeping obligations. Depending on your service, this may include your name, date of birth, contact and address details, necessary health-card information, injury and medical history, relevant doctor or emergency-contact details, work and claim information, insurance and plan-member details, uploaded documents, signatures, consent, arrival/check-in details and, where requested for the clinic visit, vehicle licence plate information. It may also include relevant communications. Not every category is needed for every patient. We explain collection purposes and obtain consent or rely on other applicable authority. Unrelated optional uses require their own appropriate process.
Our systems and cross-clinic support
Jane holds our complete clinical charts. Jane and ClinicMaster support patient information and appointments. Our internal Pure Life software supports administration and selected records, including intake information, uploaded documents, appointments and payroll-related evidence. Staff may support all three clinic locations, including the clinic operated by the other company named on this page. Cross-company information handling must have an authorized purpose and appropriate protections; sharing software does not itself authorize every disclosure.
Service providers and recipients
Authorized staff, practitioners, service providers, insurers and other recipients may handle information as needed for the stated purposes and with appropriate authority. Some supporting services process information outside Canada. We do not promise that every record or processing step remains in Canada. Ask our Privacy Officer about the handling relevant to your service.
Protection, retention and your choices
We use administrative, technical and physical safeguards appropriate to the information. No system can promise absolute security. Records are retained according to applicable clinical, financial and other requirements and legal holds. You can ask about limits on use or disclosure and withdrawal of optional consent. We explain the consequences and applicable limitations; withdrawal or account removal does not necessarily permit deletion of required existing records.
AI and recording
Optional recording and clinical AI require their own approved explanation and appropriate consent process before use. This notice is not blanket consent to recording, model training or unrestricted AI processing. Where an optional AI documentation service is offered, you may choose manual documentation without reducing your care. Staff must review AI-generated administrative or clinical drafts within their authorized roles; AI does not replace the treating clinician's responsibility.
Access, corrections and complaints
You may request access to your personal information, correction of errors and information about its use or disclosure, subject to applicable law. We verify identity or representative authority proportionately and explain any lawful limitation. You may raise questions or complaints with our Privacy Officer and seek review through the BC Office of the Information and Privacy Commissioner where applicable.

BC Office of the Information and Privacy CommissionerStaff privacy information
We handle information reasonably needed to establish, manage or end the work relationship and meet applicable obligations. Depending on your role, this includes identity and contact details, qualifications, access assignments, scheduling, clinic QR attendance and work time, compensation, training, relevant leave or personnel information, and authorized system activity.
Authorized HR, payroll, management, technical support and service providers handle information for their duties. Cross-clinic patient access does not entitle a coworker to personnel or pay records. Security logging is proportionate; this notice does not authorize unlimited surveillance. Some supporting providers process information outside Canada.
Records follow their applicable retention requirements and legal holds. Closing an account does not erase required payroll evidence or clinical authorship. You may ask about your information, seek access or correction, or raise concerns, subject to applicable law. Contact physio@purelifephysio.ca and identify your company. We verify identity and explain any lawful limitation.
